This policy does not apply to information collected through our services, offline communications, or other interactions with the Company. If you are a Willamette Family client (and not someone visiting the Site for information purposes), then you may also provide us with Protected Health Information and/or certain records that constitute health information protected by the Health Insurance Portability and Accountability Act (“HIPAA”) and/or Substance Use Disorder Patient Records protected by 42 C.F.R. Part 2. Willamette Family is a Covered Entity under HIPAA regulations regarding the use and disclosure of Protected Health Information and operates programs under 42 C.F.R. Part 2 relating to the use and disclosure of Substance Use Disorder Patient Records. Our Notice of Privacy Practices explains how we may use and disclose Protected Health Information and Substance Use Disorder Patient Records and how you may exercise your rights under HIPAA and 42 C.F.R. Part 2.
